As an approved NDIS provider for plan management, Care to Excel supports participants in managing the funding of their plans by assisting with the management of their NDIS budget and tracking their funds. We help participants claim from NDIA and pay service providers, providing them with the flexibility to choose their preferred services while remaining in full control of their funds

Participation in Community, Social & Civic Activities

Community participation support helps enable participants to engage in community and social activities. Such activities have many potential benefits for participants to overcome some of the barriers to fully participate and remain connected in the community. Development of Daily Living & Life Skills

At Care to Excel, we recognize the importance of developing daily living and life skills for our participants. Our focus is on training individuals to increase their ability to live independently and participate in community activities. This includes essential activities such as showering, toilet training, and personal care, as well as more complex tasks like shopping for groceries, planning meals, and performing household chores like washing clothes and vacuuming.

Our trained support staff work with participants to develop problem-solving skills, plan public transport journeys, and build overall confidence and independence. We understand that each participant’s needs are unique, and we take an individualized approach to support. We engage with participants and their families or carers to understand their goals and facilitate supports to help develop everyday skills. Innovative Community Participation

Care to Excel offers range of innovative community participants programs that are tailored to participants’ goal. These activities focus on participants learning, community engagement the other activities that are of participant’s interest. Assist Daily Tasks/Shared Living

At Care to Excel, we understand that some participants require assistance and/or supervision with their daily life tasks in a group living environment. We offer support services to participants living in a variety of settings, including shared households, cluster units, or host family arrangements. Our trained support workers provide personalized assistance to help participants maintain or increase their independence.

We recognize the importance of promoting the development of autonomy and independence for our participants, and our services are designed with this in mind. If you or your loved one are living in a similar setting and require support, please don’t hesitate to contact us to discuss your needs. Our team is available to help and work with you to develop a customized support plan.

Nursing Care

At Care to Excel, we understand that some participants have complex individual health and support needs. To support these participants, we have a team of highly trained and experienced registered nurses who provide specialized services, including:

  • Continence Assessment
  • Complex Wound Management
  • Medical needs/procedures such as ventilation or home enteral nutrition.
  • Complex Bowel Care
  • Enteral (Naso-Gastric Tube – Jejunum or Duodenum) Feeding and Management
  • Urinary Catheter Management (In-dwelling Urinary Catheter, In-out Catheter, Suprapubic Catheter)
  • Subcutaneous Injections
  • Day-to-day management of medications
  • Urinary Catheter Management (In-dwelling Urinary Catheter, In-out Catheter, Suprapubic Catheter)

Assist Travel Transport

We provide you with safe, convenient transport, scheduled in advance to help you get to your appointments, activities in the community and excursions. Example of transport services include (but not limited to):

  • Educational facilities
  • Shopping Centers
  • Medical and other appointments
  • Communality participation

Support Coordination

At Care to Excel, we offer assistance with coordinating and managing life stages, transitions, and supports. Our approach is personalized for each participant, taking into account their unique abilities and needs. Our service includes both short-term and long-term supports, focused on improving participants’ ability to coordinate their support services.

Our aim is to help participants live better in their homes and engage more fully with their community. By offering this service, we can help participants navigate the complexities of the NDIS and maximize the benefits of their plans.

Assist Personal Activities/Assist Personal Activities High

This support category relates to assisting with or supervising personal tasks of daily life to enable the participant to live as autonomously as possible. These supports are provided individually to participants and can be provided in a range of environments, including the participant’s own home. Daily personal activities include personal tasks of daily life such as.

  • personal hygiene, including showering, bathing, oral hygiene, dressing and grooming;
  • toileting, bladder and bowel management and menstrual care;
  • eating and drinking;
  • attending appointments;
  • use of aids and appliances, hearing and communication devices;
  • mobility and transferring, for example moving in and out of bed and on or off the toilet; or
  • application of splints, basic first aid due to injuries sustained as a result of a participant’s disability.

There are two levels of support categories for people requiring assistance with daily activities:

Standard and Higher Intensity Assist Personal Activities. High Intensity category applies when specialised, more skilled or more experienced support workers are required to provide supports to some participants with such complex needs. Examples of High intensity needs may include

  • Frequent (at least 1 instance per shift) assistance is required to manage challenging behaviours that require intensive positive behaviour support and/or
  • Continual active support is required due to high medical support needs (such as unstable seizure activity or respiratory support)
